Packing Instructions For:1 Gallon Dental Amalgam Mail-back Kits
Kit Includes: 1. (1) 1 gallon white container and lid with a gasket. 2. (1) 3 mil poly plastic bag. 3. (1) Red zip tie for the bag, located on the lid. 4. (1) Brown return shipping box. SAVE THIS BOX FOR RETURN SHIPPING. 5. (1) Strip of release tape to secure the container lid. 6. (1) Bottles Disinfecting Solution. 7. Vermiculite absorbent material. 8. (2) Zip lock bags. 9. (2) Eight ounce Poly Jars. 10. “Acceptable/Non Acceptable Material List” for the products that these kits can handle. 11. Material Safety Data Sheet. 12. Certificate of Sterilization. 13. FedEx Ground Package Return Program (PRP) – yellow instructions. Instructions: 1. Place amalgam or amalgam containing component in the white jars. 2. When jars are approximately ½ full, pour ½ of disinfecting solution in jar and seal. If all the disinfectant is not used, reseal the jar and place it in the white container. 3. Place small jars in the zip lock bag, seal and place into white container. 4. Spread vermiculite around the zip lock bags in white container. 5. Replace lid provided securely on the container. 6. After securing lid, place tape from the lid to the side of the container. 7. Place container in clear plastic bag provided and secure with the red zip tie provided. 8. Place bagged container in brown corrugated box provided. 9. Place MSDS sheet in the box prior to closing. 10. Seal the top of the box with packing tape. 11. Fill out the FedEx “PRP” return label. This information is Mandatory. You will receive a Certificate of Recycling (upon request) from the information on the label. 12. Call the 800 number on the box and FedEx will pick up the next business day. 13. Fill out and sign Certification of Sterilization and place in brown return box..
